Claims Returned Form

If you believe an item charged to your account has been returned, searches for these items will be performed by library staff once a week for three weeks. Searches are performed at both the owning branch and the location where the item was returned (if they differ). You will be notified by e-mail when the item is either located or the search is completed.

Patrons wishing to initiate searches for more than 3 items at one time must call Hayden Library Circulation at (480) 965-3605.

If the search is unsuccessful and the item is not located, an appeal may be filed with the billing department to petition to have the item discharged without being assessed a replacement fee. Instructions for filing an appeal can be found online at
